    krysallis and your krysallis therapist will ask for data and information. We will ask for your name, DOB, GP, a person to contact in an emergency, and your contact details. The krysallis therapist will also keep brief and anonymised session notes. This information is required to enable us to provide a safe and effective service.

    Data and information will be stored securely, anonymised session notes are stored separately from identifiable data.

    Data will be stored for up to 7 years following the end of therapy, or up to 7 years following the 18th birthday of a young person. All stored data and information will be destroyed at this time.

    krysallis and your krysallis therapist will at all times comply with the requirements of the Data Protection Act 1998 and General Data Protection Regulation 2018. krysallis are registered with the ICO.

    We endeavour to keep all data and information collected confidential. The exceptions to this are:

    • Where national security or serious criminal activity is involved.
    • Where there are safeguarding or child protection concerns.
    • Where you or someone else is at significant risk of harm.
    • Where we are subpoenaed by a court.
    • All krysallis therapists undergo clinical supervision, to ensure best practice. They may discuss your case within supervision, your anonymity will be protected.
    Please note that any information shared in these exceptions will only be on ‘a need to know basis’.

    Accessing your data:

    • You have the right to apply for access to any records kept about your care, support, and treatment (Subject Access Request). This request must be made in writing. Parental requests for records will be subject to the young person’s permission.
    • You have a right to request that your records are deleted at any time; this is reviewed on a case by case basis and where it is deemed that krysallis has a ‘legitimate interest’ in keeping your records in line with legislation we will inform you of why your request may be denied. This request must be made in writing.
